Birth Certificate
Medicare card
Most recent school report and most recent NAPLAN report
Visa/Passport/Naturalisation Certificate if born overseas (if applicable)
Parent/Carer Health Care Card number (if applicable)
Specialist Reports (if applicable)
Court Order/Restriction documentation (if applicable)
Ambulance Membership Number (optional)
Sacramental certificates (optional)
